Transitional Case Worker
We are currently seeking a Full-time Transitional Case Worker for Gallagher Centre. As a member of this interdisciplinary team you will provide referral and intake management, crisis intervention, discharge planning and support services for clients who are involved in the probation system or who have Mental Health Crisis issues. You will be expected to assess clients’ skills and needs, particularly in regard to their mental health and/or addiction related issues, in addition to advocacy for clients involved in the criminal justice system. You will liaise with other services i.e. probation officers, clinical support, courts to coordinate care for clients. This role will be focused on successful reintegration of clients within short time frames, completion of bi-weekly Individual Service Plans and daily check ins with clients to manage goals.
In addition to the clinical elements of the position, you will be responsible for the safety and security of the clients and other staff during your shift. You will work collaboratively with other team members to complete administrative functions for Gallagher Centre.
You will be skilled at working with clients from diverse populations; contributing to program development; as well as providing education support and supervision to students. This position is primarily located at Gallagher Centre but will include regular travel between SLCS locations and within the community.
Required Qualifications
- University Degree or 2 – 3-year College Diploma from an accredited institution
- Minimum of 1-year relevant experience preferably within justice, corrections, mental health and/or addictions
- First Aid /CPR with AED – Level C
- Valid G Class Driver’s License
- Candidates who are proficient in both official languages of Canada will be considered an asset
